5. Transfer Certificate/Migration Certificate (Original).
6. High School/Intermediate/Diploma (for lateral entry)/Graduate or any other equivalent examination certificate and mark sheets (Original & two sets of photocopy).
7. Ten passport size latest color photographs
8. Character certificate of the institute last attended.
9. Medical certificate issued by CMO.
10. Domicile certificate in the name of student.
11. Income certificate in the name of student.
12. Caste certificate in the name of student (Applicable for SC/ST/OBC only).
13. Gap Affidavit (Original).
14. Affidavit on Rs. 10/- Non-judicial stamp submitted by student & parent/guardian both regarding Anti-ragging.

1. I hereby declare that all the documents produced by me are true and original to best of my knowledge and belief. If at any stage any information furnished by me is found incorrect my candidature shall liable to be rejected.
